The government has upgraded the National Health Security System's “30-Baht Gold Card” to cover surgery for patients with severe obesity. It emphasizes that eligibility must meet medical indication criteria and the surgery is not for cosmetic purposes.
On 22 July 2026, Ms. Ploytale Lakshamisangchan, Deputy Spokesperson of the Prime Minister's Office, disclosed that the National Health Security System, known as the “30-Baht Gold Card,” enables over 47 million Thai people to access medical care equally, covering everything from common illnesses to expensive chronic diseases. Surveys show very high public satisfaction with the “30-Baht Gold Card,” viewing it as a cost-effective policy that reduces financial burdens and facilitates easier access to public health services.
Ms. Ploytale stated that to broaden coverage, the “30-Baht Gold Card” now includes bariatric surgery for patients with severe obesity meeting medical indication criteria. This aims to help reduce weight, lessen severity of comorbidities, and lower the risk of complications affecting health and quality of life. The surgery is a medical treatment, not cosmetic, and is not suitable for all obese individuals. Patients must undergo academic evaluation considering BMI, comorbidities, treatment history, physical and mental readiness, and ability to care for themselves and follow post-surgery treatment. Initially, eligible patients are between 18 and 65 years old and meet obesity criteria.
“Patients who meet the obesity criteria must regularly attend at least 80% of behavioral modification counseling sessions scheduled by doctors over six months before surgery. Those deemed appropriate for surgery will receive it at qualified facilities staffed by surgeons trained in obesity surgery and supported by multidisciplinary teams. Bariatric surgery is a benefit for patients with medical necessity but must follow strict academic guidelines to maximize treatment benefits and ensure patient safety.”
