Starting 1 Aug, the Comptroller General's Department has issued new rules expanding rights for civil servants and their families to claim costs for osteoporosis treatment medications in cases of very high fracture risk, to ensure access to essential medicines.
On 1 August 2026, Ms. Ploytale Laksamee Saengjan, Deputy Spokesperson for the Prime Minister's Office, revealed that the Comptroller General's Department has issued new regulations allowing civil servants to claim costs for the medications Teriparatide and Romosozumab to treat osteoporosis with very high fracture risk, effective for treatments from 1 Aug 2026 onwards.
Ms. Ploytale stated that the Comptroller General's Department had previously determined that cancer and hematology drugs, along with other high-cost newly registered medicines with the Food and Drug Administration (FDA) since 1 Jan 2018, cannot be claimed from the government until they are reviewed and included in the Prior Authorization (PA) system or drug formulary as applicable. Currently, Teriparatide, used to treat patients with osteoporosis at very high fracture risk, is not on the National List of Essential Medicines, while Romosozumab was registered with the FDA on 20 Oct 2020. Therefore, to allow entitled persons and their families access to necessary, effective medications aligned with clinical practice, under appropriate medical expense claims consistent with budget constraints, it is deemed appropriate to proceed in two parts, namely:
1. Establish criteria for medical expense claims for patients with osteoporosis at very high fracture risk and set medication reimbursement rates as follows:
1.1 Teriparatide medication reimbursed at no more than 7,710 baht per pen per box.
1.2 Romosozumab medication reimbursed at no more than 16,670 baht per two pens per box.
Furthermore, the use of Teriparatide and Romosozumab must comply strictly with the Comptroller General's Department's guidelines for medication claim control to be eligible for government reimbursement.
2. Require healthcare facilities to claim Teriparatide and Romosozumab medication costs solely through the civil servant medical welfare direct payment system. If a healthcare facility issues a medication receipt, it must state "non-reimbursable medication cost," and no certificate for prescribing non-National List medicines will be issued. Entitled persons cannot submit such medication receipts for reimbursement to their employing government agencies.
