Why Have All 9 Deaths in IRONMAN Triathlon Events Over the Past Year Occurred During the Swimming Segment?

A Brazilian triathlete tragically died despite rescue teams attempting to help, who made contact but were unable to save her.


On 23 April 2026, a tragic event occurred in the triathlon community when Mara Flavia, 38, a Brazilian triathlete and fitness influencer, died during the IRONMAN Texas competition at Lake Woodlands near Houston, Texas, USA, during the swimming segment on 18 April.

The incident began when firefighters were alerted around 7:30 a.m. that a swimmer had gone missing near a buoy in Lake Woodlands.

Palmer Bucks, fire chief, revealed that officials received reports from race support staff who saw the triathlete submerge beneath the water, triggering the rescue operation.

Underwater visibility was at "zero" at that time, requiring a dive team to assist, amid difficulties caused by the crowded water surface with other swimmers still competing.

Eventually, the body was found at a depth of about 10 feet. Divers recovered the body at 9:37 a.m., it was brought ashore and declared deceased at the scene.

While the exact cause of drowning has not been officially announced, many have noted that statistics show all nine recent IRONMAN triathlon deaths in the past year (including this incident) occurred during the swimming segment.

This aligns with data collected from 1986 to 2021, reporting 171 deaths during IRONMAN events, with 122 (71.3%) occurring during swimming, mostly due to drowning and cardiac arrest.

Dr. Jeff Zankoff, an emergency physician and triathlon coach who has completed six full IRONMAN races, stated that a study of over 5 million athletes over 30 years found 135 deaths or cardiac arrests, two-thirds of which happened during swimming, while cycling accounted for 16% and running only 11%.

He explained that swimming in open water is hazardous due to factors that spike adrenaline at the race start, which can trigger arrhythmia especially in athletes with underlying heart conditions. Some athletes are unfamiliar with open water swimming and panic when encountering others, and rescue is much harder in water than on land.

Dr. Jeff advises athletes not to compete in open water or lakes for the first time without gradual preparation. Most importantly, athletes should monitor themselves and seek help immediately if feeling unwell. Nonetheless, despite these precautions, sudden death remains a challenge in triathlon and is unlikely to be completely eliminated.
